SCHOOL SPORTS PHYSICALS!

Looking for a school sports physical? NW Indiana ER & Hospital offers sports physicals for all student-athletes.

• $30 per physical
• minimal to no wait time
• no appointment needed

Students must be accompanied by a parent or guardian.

Note: No childhood immunizations will be administered. We can provide college students TDAP and Titers for an additional cost.

July 17th | 8:00am – 4:00pm

July 19th | 8:00am – 4:00pm

July 25th | 8:00am – 4:00pm

August 3rd | 8:00am – 4:00pm

August 5th | 9:00am – 5:00pm

August 7th | 8:00am – 4:00pm
