Understanding the Spike in Pediatric ER Visits After School

As the school year begins and routines fill up with homework, after-school activities, and extracurriculars, it’s important not to lose sight of your child’s health. Interestingly, the most common time for pediatric ER visits is after school — but why is that?

The After-School ER Rush

Once school is back in session, kids’ days are packed with long hours in the classroom, followed by sports, clubs, and other after-school activities. According to data, emergency room visits for children rise significantly during the school year, with over 40% occurring between 6 and 10 p.m., and more than 30% between 2 and 6 p.m. Only 25% take place in the morning.

This afternoon and evening surge in visits is often called the “after-school ER rush.”

Why Do More Kids Visit the ER After School?

1. Limited Access to Primary Care Physicians

By late afternoon or evening, most pediatricians and family care offices are closed. If a child’s symptoms worsen during the day or an injury happens after hours, the ER is often the only option. Busy schedules may also delay care. What was manageable earlier can quickly become an immediate emergency.

2. Increased Risk During After-School Activities

Sports, clubs, and recreational programs typically happen after school, and with physical activity comes a higher risk of injuries like sprains, fractures, or concussions. These incidents often prompt parents to seek immediate evaluation and treatment at the ER.

3. Symptoms Worsen as the Day Progresses

Kids sometimes try to “tough it out” during the school day, especially if they’re feeling unwell. By the time they get home, symptoms like fever, stomach pain, or difficulty breathing may have worsened. When rest and at-home remedies don’t help, the ER is often the next step to getting them back to feeling better quickly.

Some of the most common illnesses and injuries treated after school hours include:

  • Falls and playground injuries
  • Sports-related injuries
  • Cuts and lacerations
  • Fever and signs of infection
  • Stomach pains
  • Asthma or other breathing difficulties
